SFCCN Enrollee Information
Eligibility
To be in the SFCCN, you must be Medicaid eligible and live in Miami-Dade or Broward. To apply for Medicaid, go to www.myflorida.com/accessflorida or call the Department of Children and Families (DCF) at 1-866-495-4295.
Children with special healthcare needs who are Medicaid or Florida KidCare eligible can be in the CMSN.

To apply for Florida KidCare, go to www.floridakidcare.org or call 1-888-540-5437.
When applying for Florida KidCare, you will need to provide the following:
- Your family's most recent tax return (Form 1040) OR
- Your Wage and Earning Statement (W-2) Form OR
- Current Paystubs (covering the last 4 weeks)
- The cost to add your child or children to employer health insurance coverage, if offered
- Your children's Social Security Number or the date applied for if you have not received a Social Security Card
When you apply for Medicaid or Florida KidCare, you will need to work with Children's Medical Services (CMS) to find out if your child with special healthcare needs can be in the CMSN. Enrollment
Most people on Medicaid will pick a health plan. It is your choice to be in the SFCCN. If you do not pick a plan, Medicaid will make that choice for you. You will have 90 days from the date you pick a plan or are assigned to one to disenroll without cause. You will then remain in the plan for the remainder of the year, until the next Open Enrollment unless you have a good cause reason to disenroll prior to that time. Enrollment choices and changes are made in Broward through the Medicaid Choice Counselor and in Miami-Dade through Medicaid Options.
